GRANT APPLICATION FORM

Please note thot only 501(c) (S) tax-exempt organizations in good standing with the
IRS that support women and/or children in North Alabama are eligible for a grant.

Applications are only accepted online and must be completely filled out to be
considered. You may also attach a single PDF document (up to two pages) of
supplemental information for consideration.

The grant application should be submitted with the signature of the organization’s
Chief Executive officer, Chairperson of the Board, or other authorized representative of the organization.

Application due by September 30th, 2023.

In what capacity is your agency and its board able to help promote the event?

Can your agency help obtain volunteers to work the event? Selected charities will be asked to provide up to 10 volunteers. Volunteers from each charity are assigned a two hour block between 5:45 p.m. and 12:15 a.m. the day of the event.

Who will benefit from the project and how many individuals will potentially be impacted? What geographical scope do these individuals represent?

How long will the project run? If beyond this year, how do you plan to support its continuation?

What are the anticipated costs of the project? Please provide a budget of anticipated expenses. Please provide an itemized project budget including other sources of support in hand or anticipated.

If your agency is chosen, can you provide a short video clip about your agency?
